As our list of insurance providers frequently changes, we recommend that you verify your benefits prior to scheduling a visit.  If your provider is not listed below, we will be glad to make inquiry about payment under your plan.
Please contact Vicky Furlong, our Surgical and Benefits Coordinator, regarding your specific type of insurance, at either (415) 923-0992x128 or via e-mail.  Whether you call or e-mail, please let her know the following:

• Your name
• Your home telephone number
• Your date of birth (so that she can confirm your identity to the insurer)
• The name of your primary healthcare insurance provider
• The date of the injury
• A brief description of the issue

At the time of this posting, Hand & Microsurgery Medical Group, Inc., is in contract with the following insurers:

• Private Healthcare Systems (PHCS)

We are not in contract with, and therefore cannot accept insurance coverage from, the following healthcare insurers:

• Blue Cross

• First Health
