Patient Information First Visit

Contact our receptionist during office hours at (415) 923-0992 or schedule a follow-up appointment as you leave our office. If you must cancel an appointment, please call at least 24 hours in advance, so that we may accommodate another patient.

We know that your time is important, and strive to see patients at their scheduled appointment time, but due to the complexity and unpredictability of our patient's problems, unavoidable delays may occur. We appreciate your patience and understanding at these times.

  • Medical Forms
  • What to Bring
    • To ensure your appointment goes efficiently, please bring the following items with you:

      • Photo ID
      • Your current valid insurance card
      • Referral authorization from your primary care provider if necessary
      • List of all medications, including dosage taken currently
      • All previous x-rays, MRI scans, or other test results relating to your medical problem
      • Previous medical records relating to current medical problem
  • Insurance
    • We accept most major insurance carriers. For more information or to determine what your health plan covers, please call our office. We are happy to verify your plans specific benefits.

      When you come for your first visit, please have your insurance information available. If your insurance requires that you have a referral form from your primary care physician, you must present it at your appointment. If you do not have the required items, your evaluation may be delayed, and you may be asked to reschedule your appointment.

  • Fees and Payments
    • Our fees vary, depending upon the service provided. Charges for x-rays and other services are in addition to the physician fees.

      We require that patients pay for the visit and treatment at the time of service. Our business office accepts cash, check, Visa, Mastercard and American Express. If you anticipate difficulty making your payments, please contact our billing department to discuss a payment plan.

  • Worker's Compensation
    • If you have worker's compensation coverage, the proper authorizations are obtained before your appointment. Our office can help expedite the process. You may be required to complete additional forms when you arrive at your appointment.

      If you need an interpreter, please inform us so that one can be scheduled.

  • Surgery
    • All surgeries are performed at Post Street Surgery Center.

      This is an outpatient surgical facility.

      Prior to your surgery, a nurse from Post Street Surgery Center will call you to review your medical history in preparation for your surgery.

      You will usually be asked to arrive 1 hour prior to your surgery to allow time for registration and prepare for your surgery. This preparation includes starting an IV, changing into appropriate surgical clothes and meeting with the anesthesiologist and your surgeon, Dr. Gordon.

      After your surgery, you will be transferred to a post-operative recovery room until you are fully awake and comfortable. You will also be given a snack prior to your discharge. Once you are ready to go home, you will be given post-operative instructions from Dr. Gordon and his staff.

      A post-operative follow-up visit will be scheduled for you on the day of your surgery. In some cases, your post-opeative follow-up visit will be scheduled with a hand therapist.

  • Pre-Op Instructions
    • Do not eat or drink anything after midnight, the night before your surgery. This includes water and gum. If it is necessary for you to take medication, by the direction of your physician, do so with only a sip of water.

      Please arrange for a responsible person to drive you home after surgery or accompany you home. You will not be allowed to leave alone and unattended.

      Stop taking any anti-inflammatory medications at least 72 hours prior to surgery. Aspirin should not be taken at least 12 days prior to surgery. Notify Dr. Gordon if you are taking any blood thinners. If you take insulin, consult with your primary care physician.

      Notify Dr. Gordon's office or the Post Street Surgery Center staff about any changes to your health (such as a cold, fever, runny nose, cough, diarrhea, etc.) to determine if your scheduled surgery needs to be rescheduled.

  • Day-Of Instructions
    • Please arrive one hour prior to your surgery.

      Please bring your insurance card and method of payment for any monies due at the time of service. We accept cash, check, and credit card (MasterCard, Visa, and American Express). Bring a photo ID as well as your prescription card if you have one.

      Please shower prior to surgery with soap and water. Remove any make-up, nail polish, lotions and oils. Remove all jewelry including body piercings.

      Wear loose comfortable clothing for easy dressing after surgery. Please leave all valuables at home. Please do not bring anyone under the age of 16.

  • Post-Op Instructions
    • It is not unusual to feel a little sleepy, lightheaded or dizzy several hours after surgery.

      Do not drive, smoke, drink alcoholic beverages, or operate machinery for 24 hours.

      Your surgeon will prescribe pain relief medication, as needed, for at-home use following surgery.

      You will be given post-operative instructions. The nurses will review these with you and answer any questions.
